Loan Forms and Federal Loan Requirements

The tabs listed below contain all of the loan forms borrowers will need in order to manage their loans. The tabs also contain the federal requirements for obtaining federal loans. Borrowers are expected to complete only those forms and requirements that are listed under their specific borrower type. For example, undergraduate students cannot submit forms or complete requirements that are listed under the "Parents" tab, and vice versa.

Please note: It is a federal violation to complete forms or requirements for a loan of which you are not the primary borrower.

Please note: the submission of your FAFSA, parent and/or student tax returns and W2s, and verification form represent your application for and interest in receiving federal student loans. Thus, there is no form listed below that is required solely to express your interest in receiving federal student loans.

Aid Request Form
If you would like to request a change to your federal student loans (Direct Subsidized and Unsubsidized Loans or Perkins Loan) or to change your work study award, please email your counselor with your requested change.
Summer Aid Request Form
Undergraduate enrolling in summer courses who are interested in federal or alternative loans to cover the costs of these courses must complete a Summer Aid Request Form for the 2009-2010 academic year. The form will be available for summer 2010 classes after April 1st, 2010 in FAOnline.

Thus, there is no form listed below that is required solely to express your interest in receiving federal student loans.

If you would like to request a change to your federal student loans (Direct Subsidized or Direct Unsubsidized Loans) or alternative loan, or to change your planned credit-hour registration, please email your counselor with your requested changes.
Federal Direct Graduate PLUS Application
Graduate students can apply for a Federal Direct Graduate PLUS loan. Borrowers must have also applied for and received a Federal Direct Student Loan in order to qualify.Grad PLUS Application 2009-10.
Graduate PLUS Action Form
Graduate students who have already applied and been approved for the Graduate PLUS loan may make adjustments to the award amounts. Please return this form to our office for review.Graduate PLUS Action Form 2009-2010.

Federal Direct PLUS Application 2009-10
Parents of undergraduate students can complete this application to apply for a Federal Direct Parent PLUS Loan for the Fall 2009, Spring 2010, and/or the Summer 2010 semester(s). Please note that this form requires a parent signature. Parent PLUS Application 2009-10.
PLUS Action Form 2009-10
Parent borrowers who have already applied and been approved for the Federal Direct Parent PLUS Loan may make adjustments to their award amounts. Please use this form if you would like to make a change to the Fall 2009, Spring 2010, and/or the Summer 2010 semester(s), and return it to our office for review. PLUS Action Form 2009-10.
