We recommend filling out as many of the following forms as are necessary online (if available).
Domestic Freshmen
- Part 1 — submit online / view PDF
- Application Supplement for Freshmen (Common Application)
- All freshman applicants must fill out this supplement which can be used for the Common Application. Submit CA supplement online or view freshman supplement
- Art and Athletic Supplements (Common Application)
- If you wish, you can also send us information on your artistic or athletic achievements: CA Art Supplement; CA Athletic Supplement
- Combined-Admission Programs (GEAR, GRADE, REBS, REMS)
- To apply for one of Rochester's Combined-Admission Programs (Graduate Engineering at Rochester, Guaranteed Rochester Accelerated Degree in Education, Rochester Early Business Scholars, or Rochester Early Medical Scholars), you must also complete this additional supplement. - Common Application First-Year Application
- Submit Common Application (CA) online or view CA PDF
- Dual Degree with Eastman (DDE) Supplement
- First-year students intending to major at both the Eastman School of Music and the College must submit this form.
- Dual Degree in Nursing (DDN) Supplement
- First-year students intending to major at both the School of Nursing and the College must submit this form.
- Early Decision Agreement (Common Application)
- First-year students applying for early decision must complete this form: CA ED confirmation
- Midyear Report (Common Application)
- As soon as your first-semester senior grades become available, please send us this form: CA Midyear
- Reactivation Form
- Only applicants who are applying within one academic year of their original application should use this form. All others must complete a new application.
- School Report (Common Application)
- Give a copy of this form to your guidance counselor or other school official: CA School Report
- Teacher Recommendation (Common Application)
- Give at least one of your teachers a copy of this form to fill out and submit to us: CA Teacher Recommendation
Transfer Students
- Part 1 — submit online / view PDF
- Application Supplement for Transfers (Common Application)
- All transfer applicants must fill out this supplement which can be used for the Common Application. Submit CA supplement online or view transfer supplement
- College Instructor Evaluation (Common Application)
- Give a copy of this form to one or two of your college instructors.
- College Report (Common Application)
- Give a copy of this form to a dean or other college official who has access to your disciplinary record and to your academic record.

- MCC 2+2 Intent to Enroll Form
- If you are a Monroe Community College student enrolling in the 2+2 Dual Admission Program, please submit this
form the semester before you enroll at Rochester.
- Mid-Term Report (Common Application)
- If we've asked you to submit your midterm grades, please use this form.
- Reactivation Form
- Only applicants who are applying within one academic year of their original application should use this form. All others must complete a new application.
International Transfer Applicants
- All international applicants must:
- complete the International Financial Support Form.
- submit the World Education Services (WES) Transcript Evaluation
(WES will send a copy to us).
